Click on the MyPortal link at the top of your page.
Complete the enrollment form. Your Medical Record Number is required, and your current email address must be on file in Medical records.
A one-time use ID, password and link to the Portal will be emailed to you.
Click on the link in the email to access the Portal.
Enter your one-time use ID, password and security question and click Log On.
Enter your new log-on user ID and password.

To self-enroll, you must use your medical record number. You may find your medical record number on your patient armband or paperwork. It is the nine-digit number following M (Example – M000987654). If you don’t know your medical record number, you may call the Medical Records Department at (706) 845-3958, from 7 a.m. to midnight on weekdays and from 8 a.m. to 4:30 p.m. on Saturdays and Sundays, and they will provide the number to you.
